Barry County Food Inspections

The Environmental Services division of the Barry County Health Department inspected 42 food-service establishments during the month of August. Of that group, 29 establishments had critical violations and six had no critical or non-critical violations.

Facilities having no violations included: Central Crossing Senior Center and Shell Knob #78 School District in Shell Knob; Sonic Drive-In and Whitley Pharmacy in Cassville; Modern Variety in Monett; and Exeter School District.

BBQ Station and Kwik T's in Cassville and Dollar General Store in Monett were inspected in August, and only non-critical violations were found.

Critical violations were found at 29 establishments:

* Cr-ose Café in Washburn, was inspected on Aug. 6, and one critical violation was found. Raw catfish was stored below raw ground beef.

* Trojan PQ Mart in Washburn was subject to a follow-up inspection from July 29, and all critical violations were corrected.

* Country Package in Washburn was subject to a follow-up inspection from July 29, and all critical violations were corrected.

* Pizza Hut in Cassville was inspected on Aug. 6, and four critical violations were found. No one ServSafe qualified or equivalent was on duty; pizza rings were not washed, rinsed and sanitized every four hours and bowls stored as clean had food debris in them; food not held to proper temperature on the prep table; sanitizer on the cook line was not at proper strength. Two non-critical violations were found.

Upon re-inspection on Aug. 9, all critical violations had been corrected.

* Pack 'n Go in Monett was inspected on Aug. 7, and one critical violation was found. Crawling insects were found in cabinets under the beverage station. Five non-critical violations were found.

Upon re-inspection on Aug. 15, all critical violations had been corrected.

* KFC/LJS in Monett was subject to a complaint inspection on Aug. 7 and three critical violations were found. Condiment bins were not clean; three vat sink had a leak; no one ServSafe or equivalent was on duty. Three non-critical violations were found.

Upon re-inspection on Aug. 15, all critical violations had been corrected.

* Country Fresh Market in Shell Knob was inspected on Aug. 8, and two critical violations were found. Roast beef on the slicer in the deli was not at proper temperature; bacterial growth was on the door of the deli ice machine. Two non-critical violations were found.

* Ramey Supermarket in Purdy was inspected on Aug. 9, and one critical violation was found. Raw catfish was stored below turkey and raw bratwurst patties were stored above ready-to-eat ham. Three non-critical violations were found.

* Eben Ezer Mexican Restaurant in Monett was inspected on Aug. 9 and all violations from July 30 were corrected except for ServSafe certification.

* Monett Family Restaurant in Monett was inspected on Aug. 9, and all critical violations from July 30 were corrected.

* Fastrip in Cassville was inspected on Aug. 12, and two critical violations were found. No one ServSafe certified was on duty; bare hand contact with ready-to-eat food. Three non-critical violations were found.

Upon re-inspection on Aug. 16, all critical violations were corrected.

* El Mariachi in Cassville was inspected on Aug. 12, and nine critical violations were found. Employee beverages were left uncovered on the prep table; employee handled raw meat without washing hands before or after; employee used bare hands to handle ready-to-eat tortillas; foods at prep counter were not held to proper temperature; three out-of-date containers of salsa were not thrown away; no one ServSafe qualified was on duty; two spatulas were chipped and no longer cleanable; four bottles of cleaner were not labeled. Four non-critical violations were found.

Upon re-inspection on Aug. 16, no one ServSafe certified was on duty.

* Subway (inside of Walmart) in Cassville was inspected on Aug. 12, and two critical violations were found. Inspector found an unlabeled bottle of liquid and food items not labeled with a discard date. Four non-critical violations were found.

* Lucky T's Convenience in Seligman was inspected on Aug. 13, and one critical violation was found. Air freshener and window cleaner were stored next to juice. Two non-critical violations were found.

* Mercy Hospital in Cassville was inspected on Aug. 15, and three critical violations were found. Raw bacon was stored on top of a pan of stuffing; the prep refrigerator was not holding items to proper temperature; food items was not labeled with a discard date.

* Dock and Eat in Shell Knob was inspected on Aug. 16, and two critical violations were found. A can opener blade was encrusted with debris; food items were not labeled with a discard date, not discarded or marked incorrectly. Two non-critical violations were found.

* Walmart Supercenter #914 in Cassville was inspected on Aug. 16, and all critical violations from July 26 were corrected.

* Casey's General Store #3068 in Cassville was inspected on Aug. 16, and three critical violations were found. Employee changed gloves without washing hands prior to putting on the new pair; spray nozzles at the soda fountain had bacterial growth; a can of paint was stored above pizza boxes in the washing machine/dryer area. Three non-critical violations were found.

* The Rusty Gate in Exeter was inspected on Aug. 17, and three critical violations were found. Employee handled raw meat and then ready-to-eat sandwich without washing hands; employee handled food items with bare hands; wastewater stabilization pond has no fence, weeds were overgrown around it and a grease sheen was on the layer of pond scum.

* Lai Lai Asian Restaurant in Monett was inspected on Aug. 20, and eight critical violations were found. Uncovered employee beverages were in the kitchen; rice held at improper temperature; partially cooked chicken was not at proper temperature; salad bar items were not held to correct temperature; shelving in the food prep area was soiled with food debris; no air gap for the three vat sink; fly paper used in kitchen not an approved method of pest control; sanitizing agent at server station was not at proper level. Nine non-critical violations were found.

Upon re-inspection on Aug. 23, all critical violations had been corrected.

Ramey #16 in Cassville was inspected on Aug. 21, and 10 critical violations were found. Garlic butter, packaged on-site, was not labeled concerning allergens; raw meat in the butcher shop were stored in the incorrect order according to minimal internal cooking temperatures; a sifter used to bread raw chicken was not washed, rinsed and sanitized between uses; rotisserie chicken not held to proper temperature; some food items lacked a seven-day discard date; fried chicken had no discard time label; two hams on display had broken vacuum seals; shelving in the prep area was soiled; the mop sink was leaking; cleaning agents were stored above the produce prep table. Seven non critical violations were found.

Upon re-inspection on Aug. 30, all critical violations had been corrected.

* Caribe Mexican Restaurant in Monett was inspected on Aug. 22, and three critical violations were found. Bacteria growth was found in the ice machine; sanitizing agent was too strong; uncovered employee beverages were in the server station. Two non-critical violations were found.

* Sunrise Family Restaurant in Cassville was subject to a complaint inspection on Aug. 22, and four critical violations were found. An employee was eating in the kitchen; food items had not been discarded by dates on labeled; the magnetic strip holding knives was soiled; sanitizing agent in the kitchen was too strong.

* Office Pub & Steakhouse in Eagle Rock was inspected on Aug. 23, and eight critical violations were found. Raw eggs were at room temperature; food items were not labeled with discard date; labeled food items were not discarded by date; utensil containers were soiled; inside of microwave was soiled; an uncovered employee beverage was in the pizza area; employee had bare hand contact with ready-to-eat food; raw meat stored over ready-to-eat food; raw food on counter was not held to proper temperature. Four non-critical violations were found.

* Weingarten in Washburn was inspected on Aug. 23, and six critical violations were found. Uncovered employee beverages were at the server station and in the kitchen; bare hand contact with ready-to-eat food; cooling units were not at proper temperature; no date label on some food items; sanitizing agent was not to recommended levels; fly strip in dish room not an approved method of pest control. Four non-critical violations were found.

* Monett Speedway in Monett was inspected on Aug. 23, and two critical violations were found. Uncovered employee drink was on the prep counter in the kitchen; one spatula and utensil container was soiled. One non-critical violation was found.

* Rapid Roberts in Shell Knob was inspected on Aug. 26, and three critical violations were found.

* Mazzio's Italian Eatery in Monett was inspected on Aug. 28 and seven critical violations were found. No one ServSafe was on duty; food items were stored below leaking drains from the soda fountain; some food items had no discard date; pizza pans stored below dough prep had debris in them; the soda fountain was leaking; pressure sprayer and pink liquid and squeeze bottle were not labeled; some food items not held to proper temperature.

* New Beginnings Ice Cream and Deli in Monett was inspected on Aug. 28, and two critical violations were found. Prep surface was soiled by food debris; pressure sprayer with liquid was not labeled. Two non-critical violations were found.

* Jack Henry Cafeteria in Monett was inspected on Aug. 29 and one critical violations was found. Sanitizing agent was not to proper concentration. One non-critical violation was found.

* Happy House in Monett was inspected on Aug. 29, and seven critical violations were found. Employee drinks were on the prep table; employee handles raw chicken and did not wash hands when changing tasks; raw chicken stored over crab rangoon and raw beef; cold food items were not held to proper temperature; garlic mixture extended the seven-day discard requirement; cooked chicken was not held to proper temperature; a knife stored between cold table shields heavily soiled by old food debris; Five non-critical violations were found.

Upon re-inspection on Aug. 30, all critical violations had been corrected.

* George's Cafeteria (Imperial Catering) in Cassville was inspected on Aug. 30 and three critical violations were found. Soup and gravy were not held to proper temperature; some food items were not discarded by proper date; shelves were rusted and not easily cleaned. One non-critical violation was found.

* The Stage Stop in Butterfield was inspected on Aug. 30, and seven critical violations were found. Some food items were not covered to protect from contamination; some food items not labeled with discard date; some labeled items not thrown away by discard date; no documented discard time on food items in the hot holding unit; a rotten pepper was stored in pizza refrigerator for use; counters, shelves, utensils and oven surfaces utilized for storing equipment were soiled; sanitizing agent exceeded recommended limits. A total of 12 non-critical violations were found.
