Health department conducts food-service inspections

The Environmental Services division of the Barry County Health Department inspected 43 food-service establishments during the month of April. Of that group, 25 establishments had critical violations and 13 had no critical or non-critical violations.

Cox Monett Cafeteria, Wheaton R-3 School, Utopian Bean in Purdy, Big Baldy's in Monett and The Herb Depot in Monett underwent routine inspections in April that yielded no critical or non-critical violations.

Bumpers Bar and Grill in Monett, Pop's Catfish mobile unit, Mr. B's Frozen Yogurt in Monett, Exeter Diner, The Calico Goose in Eagle Rock and Tropical Sno in Cassville underwent pre-opening inspections in April that yielded no critical or non-critical violations.

Jersey's Bar and Grill in Cassville underwent a follow-up inspection in April that yielded no critical or non-critical violations.

A plan review for a kitchen remodel was also conducted at Kings River Golf Course in Shell Knob.

Critical violations were found at 25 establishments:

* Acambaro Mexican Restaurant in Monett underwent a follow-up inspection on April 2 and one critical and no non-critical violations were found. The critical violation was: nobody ServSafe certified on duty.

* Braum's in Monett underwent a routine inspection on April 2 and two critical and three non-critical violations were found. Critical violations were: ice scoop container is soiled; and sanitizer too weak. The violations were corrected on the spot.

* Denali Dreams in Monett underwent a routine inspection on April 2 and one critical and two non-critical violations were found. The critical violation was: two containers of soup not labeled with discard date. The violation was corrected on the spot.

* The Bayou in Monett underwent a routine inspection on April 2 and three critical and one non-critical violations were found. Critical violations were: Caesar dressing, peaches, pear mix and cottage cheese held on the salad bar at 45 degrees instead of 41 degrees or below; chicken tenders, boiled potatoes, steaks and other food items not labeled with discard date; and catfish dated March 29, custard dated April 1, fajitas dated March 2 and sausage dated March 30 not discarded on discard date. The violations were corrected on the spot.

* Casey's General Store #2258 in Monett underwent a follow-up inspection on April 3 and one critical and no non-critical violations were found. The critical violation was: nobody ServSafe certified or equivalent on duty.

* Sunrise Family Restaurant in Monett underwent a routine inspection on April 4 and one critical and one non-critical violations were found. The critical violation was: sanitizer not at proper strength. The violation was corrected on the spot.

* Mazzio's Italian Eatery in Monett underwent a routine inspection on April 4 and three critical and six non-critical violations were found. Critical violations were: mugs and plates stored on surface that is not clean, containers stored on soiled shelf and utensils stored in container that has debris in the bottom; bottle of liquid not labeled with name of product; and Montreal beef not labeled with discard date. The violations were corrected on the spot.

* L&L Stores in Cassville underwent a follow-up inspection on April 8 and two critical and no non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; and rodent pellets too numerous to count found throughout the facility.

* Sonic Drive Inn in Cassville underwent a routine inspection on April 9 and three critical and four non-critical violations were found. Critical violations were: employee preparing food used their bare hands to make a burrito that would receive no further cooking; bun toaster pieces put away as clean have food and grease debris on them; and pipe below ice cream station three vat sink has broken creating no air cap.

* Taco Bell/KFC in Monett underwent a routine inspection on April 9 and one critical and two non-critical violations were found. The critical violation was: chicken, potato wedges, hot line and cold line for Taco Bell products past their discard time. The violation was corrected on the spot.

* Prime Cut in Monett underwent a routine inspection on April 9 and three critical and four non-critical violations were found. Critical violations were: potentially hazardous foods past seven-day discard date, including macaroni salad dated April 6; coleslaw dated March 22 and potato salad dated April 4; potentially hazardous foods not labeled with seven-day discard date, including ribs and beef; and meat slicer not cleaned within four hours of use and has dried food debris on it. The violations were corrected on the spot.

* KFC/LJS in Monett underwent a routine inspection on April 9 and two critical and four non-critical violations were found. Critical violations were: hush puppy batter past discard time; and utensils and rice stored in soiled containers. The violations were corrected on the spot.

* La Michocana in Monett underwent a routine inspection on April 9 and six critical and six non-critical violations. Critical violations were: Pepto Bismol, sealant and hand sanitizer stored above and near food or food service items; sanitizer not at proper strength; meats in display case not labeled with seven-day discard date; no water at hand sink; faucets at three-compartment sink not working; and shelves and prep tables in meat department are not clean. A follow-up inspection was conducted on April 12 and a pre-opening permit was issued to the establishment.

* Grant's Restaurant in Monett underwent a routine inspection on April 11 and four critical and two non-critical violations were found. Critical violations were: third compartment of the three-vat sink has a leak; grill in kitchen is soiled by food debris and grease; apple corer/slicer is broken and repaired with masking tape; and knife sharpener stored in a drawer with medicines, such as laxatives an burn ointment. A follow-up inspection was conducted on April 16 and all critical violations were corrected.

* Rapid Robert's in Monett underwent a routine inspection on April 11 and three critical and one non-critical violations were found. Critical violations were: bacteria growth in the ice machine, food spill on the bottom shelf of the mini refrigerator and lid dispenser for coffee lids is soiled; glass cleaner stored with straws; and nobody ServSafe certified or equivalent on duty. A follow-up inspection was conducted on April 16 and all critical violations were corrected.

* Monett Elementary School underwent a routine inspection on April 11 and two critical and one non-critical violations were found. Critical violations were: uncovered employee beverage in dry storage; and tuna salad and shredded cheese on the salad bar held at 51 degrees and 49 degrees instead of 41 degrees or below. The violations were corrected on the spot.

* Happy House in Monett underwent a routine inspection on April 11 and three critical and four non-critical violations were found. Critical violations were: raw beef stored above cooked chicken; a wok and the cart it was stored on have food debris on them when stored as clean; and sanitizer too weak. The violations were corrected on the spot.

* George's Cafeteria in Butterfield underwent a routine inspection on April 12 and three critical and one non-critical violations were found. Critical violations were: gravy and taco meat held in hot hold unit at 119 degrees and 129 degrees instead of 140 degrees or above; fruit bowls, cottage cheese and cottage cheese with fruit not discarded on discard date of April 11; and fried zucchini and chicken sandwich held by time as a public health control has no documented discard time. The violations were corrected on the spot.

* Jackson Brothers of the South, Tyson Cafeteria in Monett underwent a routine inspection on April 12 and one critical and two non-critical violations were found. The critical violation was: fried chicken non marked with discard time. The violation was corrected on the spot.

* Country N in Wheaton underwent a routine inspection on April 16 and eight critical and 11 non-critical violations were found. Critical violations were: kraut salad not discarded on discard date on April 6; shelving for food and food related items in kitchen by steam table is heavily soiled with food debris, hair, dead bugs and dust and knife, ladle and measuring spoons stored as clean have food debris on them; degreaser stored by plastic food bags on top of freezer and insecticides, toxic sealant and cleaner stored with food in dry storage; unsifted breading used for raw chicken on previous day stored uncovered in dry storage; eggs that are not from an approved source in walk-in cooler, if for personal use, must be labeled as such; molded, rotten lemons in walk-in cooler; food prep surfaces wiped with soapy water but not sanitized; and three-compartment sink leaking. A follow-up inspection was conducted on April 23 and all critical violations were corrected.

* Country Corner in Purdy underwent a routine inspection on April 16 and three critical and four non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; fried foods in hot case held by time as a public health control have no documented discard time; and four plastic spoons melted causing them to have rough edges and creases, must be smooth and easily cleanable.

* Plaza Mexico in Monett underwent a routine inspection on April 18 and eight critical and four non-critical violations were found. Critical violations were: employee beverages in the meat area have no lid or straw; employee put on gloves to fill order for a customer, but did not wash their hands prior; raw chicken stored on top of cheese, salsa and ready-to-eat food; sour cream, salsa and cooked cactus made and packaged in the establishment not dated with the seven-day discard date; Adobe sauce dated April 14 not discarded on discard date; raw chicken thawing in a sink soiled by food debris; slicers in the meat area soiled and not cleaned at least every four hours; and sanitizers in meat area too weak. A follow-up inspection was conducted on April 19 and all critical violations were corrected.

* The Rusty Gate in Exeter underwent a routine inspection on April 20 and one critical and three non-critical violations were found. The critical violation was: sanitizer too strong. The violation was corrected on the spot.

* Monett Speedway underwent a routine inspection on April 21 and one critical and two non-critical violations were found. The critical violation was: nobody ServSafe certified or equivalent on duty.

* Mexico Restaurant 2000 in Monett underwent a routine inspection on April 26 and six critical and one non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; raw meat stored above lettuce in refrigerator; cheese held at 52 degrees, diced tomatoes held at 57 degrees, salsa held at 75 degrees and lettuce, mayonnaise and individual salsa held without time or temperature control; chicken mix dated to be discarded April 25 not discarded; dishes washed and rinsed but not sanitized; and can opener blade soiled.

The following five establishments had only non-critical violations:

* Monett Middle School underwent a routine inspection on April 4 and three non-critical violations were found.

* Oakview School in Monett underwent a routine inspection on April 8 and one non-critical violation was found.

* Jack Henry Cafeteria in Monett underwent a routine inspection on April 12 and two non-critical violations were found.

* Terry's Cafe in Shell Knob underwent a routine inspection on April 19 and three non-critical violations were found.

* Wheaton Sale Barn underwent a routine inspection on April 20 and two non-critical violations were found.
