Health department conducts food-service inspections

The Environmental Services division of the Barry County Health Department inspected 48 food-service establishments during the month of January. Of that group, 31 establishments had critical violations and 16 had no critical or non-critical violations.

Big Baldy's BBQ in Monett, L & L Stop and Shop in Exeter, Simply Sweet Shoppe in Cassville, Coffee Cabin in Monett and McDonald's in Monett underwent routine inspections in January that yielded no critical or non-critical violations.

J & S Floral in Exeter, The Rusty Gate Cafe in Exeter, Eben Ezer Mexican Restaurant in Monett and Third Street Grill in Monett underwent pre-opening inspections in January that yielded no critical or non-critical violations.

Purdy's Little Diner, Jack Henry Cafeteria in Monett, Lai Lai Restaurant in Monett, Purdy R-2 School, Mexico Restaurant 2000 in Monett and Monett Elementary School underwent follow-up inspections in January that yielded no critical violations.

Happy House in Monett underwent a complaint inspection in January that yielded no critical or non-critical violations.

Critical violations were found at 31 establishments:

* Pizza Hut in Cassville underwent a routine inspection on Jan. 2 and two critical and two non-critical violations were found. Critical violations were: container clean spoons are stored in has debris in the bottom; and bucket used for ice not clean. The violations were corrected on the spot.

* Country Diner in Shell Knob underwent a follow-up inspection on Jan. 3 and one critical violation was found. The critical violation was: establishment has not yet been serviced by a licensed pest control operator; service was scheduled for Jan. 4. A follow-up inspection was collected on Jan. 4 and no critical violations were found.

* Uncle Roy's One Stop in Eagle Rock underwent a routine inspection on Jan. 3 and two critical and one non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; and ice machine has bacteriological growth inside. A follow-up inspection was conducted on Jan. 7 and one critical and no non-critical violations were found. The critical violation was: ice machine has bacteria growth inside. A second follow-up inspection was conducted on Jan. 9 and no critical violations were found.

* Fastrip in Cassville underwent a routine inspection on Jan. 4 and two critical and one non-critical violations were found. Critical violations were: employee drink on the counter in ice cream area has no lid or straw; and sweet and sour pork with discard date of Jan. 3 not discarded. The violations were corrected on the spot.

* B & J BBQ and Fish in Shell Knob underwent a routine inspection on Jan. 4 and eight critical and 11 non-critical violations were found. Critical violations were: raw turkey thawing with raw beef and raw sausage thawing with raw chicken, posing risk of cross contamination; three containers used to store utensils and knives have heavy accumulation of debris in them; pork no discarded on discard date of Jan. 3 and egg wash not discarded on discard date of Jan. 2; sanitizing solutions in kitchen and server station not at proper strength; soiled pan on top of ice machine; container ice scoop is stored in is not clean; inside of ice machine has bacterial growth; unused flat top grill with burners and oven, located on cook's line, is heavily soiled and plastic drawer unit storing utensils is soiled on the outside and has soiled utensils stored in it, needs cleaned or removed from premise. A follow-up inspection was conducted on Jan. 7 and no critical violations were found.

* VFW #4207 in Monett underwent a routine inspection on Jan. 9 and one critical and three non-critical violations were found. The critical violation was: nobody ServSafe certified or equivalent on duty. A follow-up inspection was conducted on Jan. 29 and one critical violation was found. The critical violation was: nobody ServSafe certified or equivalent on duty.

* Taco Bell #537 in Monett underwent a routine inspection on Jan. 9 and one critical and two non-critical violations were found. The critical violation was: employee beverage on the prep counter in the kitchen has no lid or straw. The violation was corrected on the spot.

* Kum & Go #492 in Monett underwent a routine inspection on Jan. 9 and four critical and two non-critical violations were found. Critical violations were: special sauce dated Nov. 6, 2012, and beef sausage and pepperoni dated Dec. 31, 2012, not discarded on discard dates; pizza, hot sandwiches and eggrolls held by time as a public health control have no discard time; soda nozzles have bacteria growth; and drain by ice machine has several hoses and pipes going down into the drain that do not have an air gap. A follow-up inspection was conducted on Jan. 25 and no critical violations were found.

* C & C Country Store in Exeter underwent a routine inspection on Jan. 10 and four critical and one non-critical violations were found. Critical violations were: two uncovered employee beverages in the kitchen and dishwashing areas; meat slicer in storage area and meat scraper in dish area on the knife holder are soiled; cleaner, De-icer and bug fogger stored on shelf above soda and water; and sanitizer for washing dishes and in deli area is too weak. A follow-up inspection was conducted on Jan. 14 an no critical violations were found.

* Ramey Supermarket in Purdy underwent a routine inspection on Jan. 11 and three critical and three non-critical violations were found. Critical violations were: uncovered employee beverage in produce prep area; meat display cases not clean, dried meat debris and dried blood; and insecticide stored above tomatoes and bananas. A follow-up inspection was conducted on Jan. 14 and no critical violations were found.

* Subway in Cassville underwent a routine inspection on Jan. 11 and five critical and no non-critical violations were found. Critical violations were: food utensils, pans, lids washed and rinsed but not sanitized; uncovered employee beverage in the food prep area; soiled measuring cup stored as clean and clean lids and pan inserts stored in containers that have accumulation of food debris in the bottoms; nobody ServSafe certified or equivalent on duty; and hot water heater is leaking. A follow-up inspection was conducted on Jan. 18 and no critical violations were found.

* Ruby's in Seligman underwent a routine inspection on Jan. 15 and five critical and nine non-critical violations were found. Critical violations were: dressings in individual cups and pasta salad not dated with seven-day discard date; grilled chicken dated Jan. 12 and pizza sauces dated Jan. 11 and Jan. 12 not discarded on discard date; burrito held in hot case at 120 degrees instead of 140 degrees or above; two spray bottles with blue liquid chemicals not labeled with common name; and pesticide spray in kitchen not approved for food service. The violations were corrected on the spot.

* Trojan PQ Mart in Washburn underwent a routine inspection on Jan. 15 and two critical and three non-critical violations were found. Critical violations were: uncovered employee beverage sitting on the kitchen counter; and cut lettuce in the kitchen pizza refrigerator not discarded on discard date of Jan. 13. The violations were corrected on the spot.

* Cassville Conoco underwent a routine inspection on Jan. 16 and seven critical and four non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; no designated person in charge; shelving in the kitchen for cans and pizza boxes have contact paper that is peeling and shelves are no longer easily cleanable; container for coffee cup lids is soiled and wire shelving in walk-in cooler for sodas and beer are soiled; pizza pans wiped with a wash cloth and put away clean; two mislabeled chemical spray bottles of chemical taken out of their original containers; and floor cleaner stored on a shelf above sugar. A follow-up inspection was conducted on Jan. 28 and one critical and no non-critical violations were found. The critical violation was: nobody ServSafe certified or equivalent on duty.

* Ramey #16 in Cassville underwent a routine inspection on Jan. 16 and 11 critical and 12 non-critical violations were found. Critical violations were: Clorox cleaner stored next to deli paper; sanitizer in bakery/deli too strong, toxic; potato salad with use by/sell by date of Dec. 25, 2012, and Chinese chicken with sell by date of Nov. 12, 2012, in walk-in cooler; ribs not labeled with discard date in deli; nobody ServSafe certified or equivalent on duty; moldy sandwich in dairy walk-in cooler; container that ice scoop is stored in is not clean; uncovered meat stored under fan guards that have heavy dust accumulation; clean food containers stored on soiled shelves in bakery/deli; three vats sink in bakery/deli not holding water; and food containers and utensils not being washed, rinsed and sanitized. A follow-up inspection was conducted on Jan. 18 and two critical and no non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; and three vats sink in bakery/deli not holding water.

* Taco Bell/KFC in Cassville underwent a routine inspection on Jan. 17 and three critical and two non-critical violations were found. Critical violations were: no discard time on one tray of fried chicken; raw chicken not discarded on discard date, chicken filets, Jan. 16 and Jan. 13 and chicken strips, Jan. 15; and bacteria growth on ice chute on soda fountain in dining room. The violations were corrected on the spot.

* El Mariachi in Cassville underwent a routine inspection on Jan. 17 and six critical and eight non-critical violations were found. Critical violations were: unlabeled spray bottles of chemicals; burn spray, antiseptic, rubbing alcohol and bottles of medicine stored with food; uncovered employee beverage on prep table while meat is being cut; pan lids stored in container that is not clean; cut tomato not refrigerated and is at 71 degrees, discarded during inspection; and nobody ServSafe certified or equivalent on duty.

* Casey's General Store #2058 in Monett underwent a routine inspection on Jan. 17 and one critical and four non-critical violations were found. The critical violation was: knives and container for knives in drawer under donut prep counter are soiled, put away as clean. The violation was corrected on the spot.

* Prime Cut in Monett underwent a routine inspection on Jan. 22 and three critical and three non-critical violations were found. Critical violations were: numerous food items without discard date; numerous food items pas their discard dates, barbecued beef, Dec. 25, 2012, and filet mignon, Jan. 12; and two hams in meat case on display for sale are spoiled.

* Grant's Restaurant in Monett underwent a routine inspection on Jan. 22 and five critical and three non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; employees did not wash their hands prior to putting on gloves; apple slicer put away as clean with food debris and dried juice on it; employee used soiled oven mitt to handle ready-to-eat food while slicing it for the buffet; and plumbing beneath three compartment sink in the dish washing area leaks. A follow-up inspection was conducted on Jan. 23 and no critical violations were found.

* L & L Stores in Eagle Rock underwent a routine inspection on Jan. 23 and two critical and two non-critical violations were found. Critical violations were: pineapple and diced onions not labeled with seven-day discard date; and utensils stored in container that has debris in the bottom. The violations were corrected on the spot.

* Country Fresh Market in Shell Knob underwent a routine inspection on Jan. 23 and one critical and two non-critical violations were found. The critical violation was no air gap between ice machine drain pipe and floor drain. The violation was corrected on the spot.

* Kwik T's in Cassville underwent a routine inspection on Jan. 24 and two critical and one non-critical violations were found. Critical violations were: sanitizer not at proper strength; and cheeseburgers in hot case held at 118 degrees instead of 140 degrees or above or with use time as a public health control. The violations were corrected on the spot.

* Plaza Mexico in Monett underwent a complaint inspection on Jan. 25 and five critical and one non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; tortilla machine not clean, has dried food debris on it; mayonnaise on counter is at 71 degrees instead of 41 degrees or below after opened; numerous raw meats not labeled with discard date; and open can of soda on meat prep table. A follow-up inspection was conducted on Jan. 29 and no critical violations were found.

* Geraldi's in Cassville underwent a routine inspection on Jan. 25 and one critical and one non-critical violations were found. The critical violation was: aujus sauce dated Jan. 10 and Jan. 21, Canadian bacon dated Jan. 24 and beef roast drippings/broth dated Jan. 20 not discarded on discard date. The violation was corrected on the spot.

* Sunrise Family Restaurant in Cassville underwent a routine inspection on Jan. 28 and six critical and five non-critical violations were found. Critical violations were: numerous foods in coolers around cook line and spices in dry storage left open and uncovered; mashed potatoes held on steam table at 125 degrees instead of 140 degrees or above and sliced cheese inside prep line cooler at 45 degrees instead of 41 degrees or below; sliced lettuce not labeled with seven-day discard date an potentially hazardous condiments, such as Bleu cheese dressing, in server coolers given more than seven days for discard date; can opener put away as clean soiled by food debris and ice scoop setting in container with soiled water; employee rinsed can opener and dried with towel then proceeded to put it away as clean; and unlabeled squeeze bottle of chemicals. The violations were corrected on the spot.

* KFC/LJS in Monett underwent a routine inspection on Jan. 29 and two critical and four non-critical violations were found. Critical violations were: employee changed gloves several times and went from receiving money to preparing food to handling chemicals without washing their hands; and spice shaker put away as clean is heavily soiled and scale put away as clean is heavily soiled. The violations were corrected on the spot.

* Subway inside the Walmart Supercenter in Monett underwent a routine inspection on Jan. 30 and three critical and three non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; pizzas with discard date of Jan. 26 not discarded; and pesticide not approved for food service used in establishment.

* Burger King in Monett underwent a routine inspection on Jan. 31 and three critical and six non-critical violations were found. Critical violations were: peppers and onion mix not discarded at required discard time, discard time was 2 p.m. and product was still on the line at 2:45 p.m.; 20 plus flies observed in kitchen; and water leaking from heat exchange unit in kitchen causing standing water on floor.

* Mocha Jo's Coffee Cafe in Monett underwent a routine inspection on Jan. 31 and two critical and no non-critical violations were found. Critical violations were: corn salad with discard date of Jan. 30 not discarded; and cut tomatoes held at 45 degrees instead of 41 degrees or below.

* Pack 'n' Go in Monett underwent a routine inspection on Jan. 30 and three critical and six non-critical violations were found. Critical violations were: nobody ServSafe certified or equivalent on duty; ice machine has bacteria growth; and utensils for pizza not washed, rinsed and sanitized between uses and employee sprayed water on soiled pizza utensils and wiped them off to clean them before next use.

The following establishment had only non-critical violations:

* Mazzio's Italian Eatery in Monett underwent a routine inspection on Jan. 3 and three non-critical violations were found.
